These forms give us general information about you and your health goals. After completing the paperwork and checking in at our front desk you will have a consultation with either Dr. Josh. During this consultation, you will discuss your health related problems, concerns, goals, expectations and possible treatment options.
Here are the basic forms for new patients. Please download and fill out the Adult History Form, Financial Policy Form, and the Informed Consent. If your child has an appointment with us download the Child History Form instead of the Adult History Form. If you have any questions regarding the forms please give us a call or we can address them at your appointment. Dr. Josh is a gentle giant. I look forward to seeing him because he helps me with my pain. And The lady at the desk is a sweetheart. So far he has been the best one in town that has actually been able to help without causing more pain like him so much. He keeps this old diesel mechanic walking. My quality of life was crap before i began visiting dr.dick. after the first visit my chronic head aches were cleared up and found a slipped tendent in my shoulder limiting motion of my arm. Since ive taken my mom who has dish disease and recommended several friends. I also still make the trip. 48 miles 1 way

I have been going to Josh for the last 3 months with lower back trouble. The first time I went there I could hardly walk when I left there I was walking again with a lot less pain with in a week he had me back to normal.
