Your payment is due at the time of service (as required by insurance companies: co-pay, co-insurance, and deductible amounts). We will automatically submit your claims to your insurance company. However, your insurance policy is an agreement between you and your insurance company. You are personally responsible for payment of all services rendered to you if your insurance does not pay. Thank you for your understanding. Our clinic requires at least 24 hours advance notice for rescheduling or canceling an appointment.
