My name is Anne, and I am Access To Health's Practice Coordinator. I recently moved to Colorado from Minnesota and am loving the beautiful sunshine here! My husband and I enjoy exploring the outdoors and cooking healthy recipes together. We have two grown children, who live in MN. I love meeting all the great patients at Access to Health and encouraging them towards more vibrant health!

We understand that arriving at a doctor's office for the first time can be nerve racking. If you've never experienced chiropractic care before, you may not know what to expect when you come to our Fort Collins chiropractic office for the first time. Filling out paperwork, learning new faces and names, and getting to know your way around the office can be an overwhelming experience.

The online intake is preferred, but we understand sometimes computers don't do what we want them to. Please complete this on a laptop or desktop computer (the form will not transmit when completed on a mobile device). You do not need to complete every entry on the form. If something does not apply, simply skip that and go to the next question. At the end, it may say fields are incomplete, but it will transmit okay!
