If you would like to receive a copy of your medical records we ask that you email us at Kingfamilychiro@sbcglobal.net with a letter asking for a copy of your doctor records. We will email them personally to you so please include an email in your request.

Most of our patient records have numerous pages we ask that we can email you a copy of your record. You may choose to have a one page doctor report written on the procedures, technique, vitamins, exercise, protocols, that have worked in the past for you.
