The New Patient

Your first visit at Lakeside Chiropractic will be your longest and most detailed. The initial paperwork will have to be filled out (necessary forms may be downloaded from the New Patient Forms section below and completed ahead of time) a family/medical history is taken, and an examination will be performed.

After seeing my family doctor for a year, getting my eyes checked twice, and then seeing a neurologist for 2 years with little relief, I went to Dr. Cindy in January '09. Dr. Cindy's radiology technician, Ginger, performed a set of x-rays of my neck which Dr. Cindy thoroughly explained to me. After my first adjustment, I had my first day with no headache in over 3 years.
