Dr. Steve will discuss any other concerns which are not included on the intake forms. This initial visit is designed to learn more about you, your condition, and expectations. That information allows the doctor to determine how chiropractic care can meet your goals. Please arrive 5 minutes early to allow time to process any paperwork, insurance information and / or payments. For payment, we accept health savings plans, flexible spending accounts as well as cash, check or charge.
